 Paramedic Course Commencement Minimize

Within 10 days of the Paramedic course commencement, instructors submit the following:

 Paramedic Course Completion Minimize

Within two (2) weeks of the Paramedic course completion, instructors submit the following:

Instructors: you must keep track of your students ride time & field summative evaluation hours.  Upon completion of these hours, submit the following:  Paramedic Training Verification Form

A paramedic license will not be issued without this form, No. 2 on the Paramedic Initial Licensure Checklist. It is required of ALL TEI's.

For students desiring to sit for the NREMT-P Written Exam in order to obtain KY Paramedic Licensure, instructors submit the following:

 NREMT - P Written Exam Minimize

Inform students taking the exam that they must bring the following to the exams:

  • $50 certified check or money order payable to the Kentucky Board of EMS
  • Photo identification

Students must send the following to the NREMT at least 3 weeks prior to exam date:

  • A $50 certified check or money order payable to NREMT
  • Completed NREMT-P application and required documentation

 Annual Summary Report Minimize

No later than July 31, of each year the EMS-TEI Course Coordinator will submit the Annual Summary Report Form.  

If no courses were taught during the last reporting period, file an Annual Summary Report stating that no courses were taught during the reporting period.

 Posting Upcoming Classes Minimize
