FR Course Commencement

Within 10 days of the FR course commencement, instructors submit the following:

• Course Notification Report updated 11/8/04
  • Please send a copy of this report with any follow-up paperwork sent in. It tells us whose class the paperwork is for.
• Course Summary Roster
  • Agency Name, Course Number, Name & SSN are all that is needed at the beginning of class.
• FR Initial Application
  • Instructors be aware that a state criminal background check from the state(s) of residency of the past 5 years should accompany application.
• Checklist
  • Instructors notify students that copies of ALL items on checklist must be submitted for KY FR Certification at the end of class.
• $10 application fee

Within two (2) weeks of the FR course completion, instructors submit the following:

• Course Summary Report (Please note anyone who withdrew or failed the course)
• Copy of First Responder Initial Application submitted at beginning of class
• Copies of all items on Checklist
  • Note - certification will not be issued without checklist items. 
• Skills Exam Sheets
  • FR Skills Exam
  • FR Skills Exam Evaluation
• $15 money order or cashiers check payable to the Kentucky Board of EMS (for KY FR certification).

For FR students desiring to sit for the NREMT-FR Written Exam in order to obtain KY FR certification, instructors submit the following:

• Course Summary Roster for only those students requesting to take the NREMT-FR Written Exam
• Student Testing Eligibility Form for each student
• Contact Amanda Hutson by e-mail, amanda.hutson@kctcs.edu, or call 859-256-3565 to schedule an exam if you have 10+ students to take the exam.

No later than July 31, of each year the EMS-TEI Course Coordinator will submit the Annual Summary Report Form.  

If no courses were taught during the last reporting period, file an Annual Summary Report stating that no courses were taught during the reporting period.
