EMT Course Instructions

EMT Course Commencement

Within 10 days of the EMT course commencement, instructors submit the following:

Course Notification Report (PDF)
Course Summary Roster (PDF)
EMT Initial Application (PDF) Instructors please be aware that a state criminal background check from the state (s) of residency of the past 5 years should accompany application.

EMT Course Completion

Within two (2) weeks of the EMT course completion, instructors submit the following:

Course Summary Report (PDF) (Please note anyone who withdrew or failed the course)
Skills Exam Sheets
- EMT Basic Skill Sheets Front (PDF)
- EMT Basic Skill Sheets Backs (PDF)
$25 money order or cashiers check payable to the Kentucky State Treasurer (for KY EMT certification)

For students desiring to sit for the NREMT-EMT Written Exam in order to obtain KY EMT certification, instructors submit the following:

Course Summary Roster (PDF) for only those students requesting to take the NREMT-FR Written Exam
Student Eligibility Form (PDF) for each student

Annual Summary Report

No later than July 31, of each year the EMS-TEI Course Coordinator will submit the EMS TEI Annual Summary Report (PDF) Form.

If no courses were taught during the last reporting period, file an Annual Summary Report stating that no courses were taught during the reporting period.

If you would like to post upcoming EMT classes on the KBEMS calendar, please send the information with the start date and any contact information you wish to have published to Brooke.Miller@kctcs.edu.
