EMT Reinstatement Requirements Checklist (PDF) is a checklist for all requirements that must be met for an expired EMT who was previously certified in Kentucky to be reinstated as an EMT. Please be sure that you complete a total of 48 hours of continuing education as shown on this checklist.  Please be aware that a criminal background check from state of residency for past 5 years will be required to be submitted with all applications.
