The EMT Reciprocity Packet is for individuals that are currently certified in another state wishing to obtain certification in Kentucky. The individuals must be currently certified by the National Registry of Emergency Medical Technicians. A money order or certified check must be included for $35.00 payable to Kentucky Board of Emergency Medical Services. EMT Reciprocity Requirements Checklist is a check list of all requirements to be met for reciprocity.  Please provide proof of all items on the checklist. Please be aware that a criminal background check from state of residency for past 5 years will be required to be submitted with all applications.

EMT Initial Application (PDF) is one of the two applications that must be completed and submitted to the KBEMS Office.  It must accompanied by the EMT Initial Application.
