For CD version of the Kentucky Ambulance Provider Directory. (Must be sent with $15.00 fee in order for request to be processed.)

Ambulance Provider Ceased Operations Form (pdf)
Used in order to close service file on a service that has voluntarily surrendered its Kentucky Ambulance Provider License Number.
