Licensure

In order to become an Licensed Paramedic in the state of Kentucky, you will need to apply under one of the following application types.

Initial Licensure (from a Kentucky Approved Course)

Paramedic Checklist (KBEMS-E6) on the forms page.

  • Create an account in the Kentucky Emergency Medical Services Information System (KEMSIS). 
  • Current CPR for the Healthcare Provider Certification
  • Completion of CHFS approved HIV/AIDS Course
    • Course Information can be found here: HIV/AIDS Course Information
      • Current List of Courses can be found here: Current Courses
      • Must NOT have been found guilty of, entered a guilty plea, or Alford plea to a felony offense or have completed a diversion program or a felony offense
    • Must NOT have been subjected to discipline that would prevent certification at teh time of application
    • Motor Vehicles Operators License (Driver's License) for a US State or Territory
    • High School Diploma or General Education Development (GED)
    • Must be 18 years of age or older
    • Current Advanced Cardiac Life Support (ACLS) Certification
    • Background Check less than 6 months old:
        • Initial Kentucky certification or licensure requires a background check from Verified Credentials.
      • Kentucky EMT-B Certification or National Registry EMT Certification
      • Detemination of Death Certification
      • The TEI where you have obtained your education must submit Training Verification and Course Completion paperwork.
      • $75.00 Fee ($65.00 Licensure Fee + $10.00 Application Fee)

      Temporary Licensure

      This licensure is for those individuals that are seeking an immediate temporary license from the Kentucky Board of EMS. This licensure allows the individual time to obtain National Registry licensure and Determination of Death class. This licensure is only valid for 6 months and is NOT renewable.

        • Motor Vehicles Operators License (Driver's License) for a US State or Territory
        • High School Diploma or General Education Development (GED)
        • Must be 18 years of age or older
        • Current Advanced Cardiac Life Support (ACLS) Certification
        • Background check less than 6 months old:
        • Current Paramedic Licensure from another state or National Registry
        • $200.00 Fees ($125.00 Temporary Fee + $65.00 Certification Fee + $10.00 Application Fee)

        Reciprocity Licensure

        This licensure is for those individuals that are seeking an a renewable licensure from the Kentucky Board of EMS. This licensure is for those individuals who are licensed in another state or licensed by the National Registry. This is a renewable license.

        • Must NOT have been subjected to discipline that would prevent certification at teh time of application

        • High School Diploma or General Education Development (GED)
        • Must be 18 years of age or older
        • Current Advanced Cardiac Life Support (ACLS) Certification
        • Background check less than 6 months old:
        • Current licensure by the National Registry
        • Current licensure by another state (optional)
        • Determination of Death Certification
        • $200.00 Fees ($125.00 Reciprocity Fee + $65.00 Certification Fee + $10.00 Application Fee)

        Reinstatement

        This licensure is for those individuals that have let their Kentucky Paramedic licensure expire. The expiration date of your previous certification cannot be more than 5 years old. (i.e. If your certification expired on December 31, 2010, you must apply for reinstatement prior to December 31, 2015). This is a renewable license.

        • High School Diploma or General Education Development (GED)
        • Must be 18 years of age or older
        • Current Advanced Cardiac Life Support (ACLS) Certification
        • Background check less than 6 months old:
        • Evidence of previous certification as an Paramedic in Kentucky
        • Verification of Competency
        • Fees Submitted $225.00 ($150.00 Reinstatement Fee + $65.00 Certification Fee + $10.00 Application Fee)
        • Continuing Education Requirements
          • must be completed within the twelve (12) months prior to application for reinstatement
          • Evidence of successful completion of the National Standard Curriculum Paramedic Refresher Course

        OR

          • Continuing Education that meets the requirements of that curriculum
